To apply to the Adult Practical Nursing program, please submit the listed documentation by June 16, 2017 for the 2017-18 Programs:
*Medical documentation: Please complete the Student Medical History Form. The entire Physical Examination Health Form must be completed, signed and stamped with the license number of the Physician, Nurse Practitioner or Physician's Assistant performing the physical exam. A certificate of immunization is also required (see requirements in the Physical Examination Form in the Application Packet) along with a two-step PPD. The Physical Examination Form is included in the Application Packet.
**Three references are required (See Application Packet). Please choose references who can speak to your ability to perform academically and skillfully (your employer, supervisor, former teachers, etc.). References from family members, friends or co-workers will not be accepted. Give one to each person you choose as a reference. Be sure your name is on it. Give each person a stamped envelope with the address of the Financial Aid Office on it. Reference letters will not be accepted unless they are delivered via US mail. (See Address – Page 3)
***The WSWHE BOCES must receive your “official” high school transcript. Transcripts must be sent directly from your high school to the Financial Aid Office. Transcripts will not be accepted as “official” unless they are delivered via US mail. (See Address – Page 3)

Who can be admitted to the Adult Practical Nursing Program?
To be admissable to the WSWHE BOCES PN Program you may pass the TEAS Exam or you may submit a qualifying college transcript (see Related Files - Admissions Standards).
If you have not done so, go to www.atitesting.com to register for the TEAS (Test of Essential Academic Skills (Pre-admission) Examination. Study guides and online practice tests are available for purchase here (Optional). TEAS Examinations will be given every Thursday during March and April. All TEAS Exams are given at the Southern Adirondack Education Center (SAEC) in Hudson Falls. (See Related Files – PN TEAS Information).
No student will be admitted to the TEAS Exam without a photo ID. Please arrive promptly and allow 4 hours for TEAS instructions and the TEAS Exam.
Students must achieve a score of 58% or higher in Reading and Math and at least 41.3% in Science or 49% in English Language usage to be eligible for enrollment in the PN program.
After passing the TEAS Exam and submitting your completed PN Application, you must attend an interview with the Adult PN Supervisor. Please prepare and dress as you would for an employment interview. All interviews are conducted at Southern Adirondack Education Center and may include up to 4 members of the PN program.
Students accepted to the PN Program will be notified by mail by June 30. To reserve placement in the PN Program, please submit a $100.00 non-refundable fee to the PN Admissions Office. Please make check payable to: WSWHE BOCES. (See Address – Page 3)
Federal Financial Aid is available to students entering the PN Program in the form of Pell, SEOG and Direct Federal Student Loans. Students must complete a FAFSA (Free Application for Federal Student Aid) to be eligible for federal aid. Students interested in applying for federal direct student loans must complete, on line, Entrance Counseling and a Master Promissory Note.

Textbooks and Supplies
To be prepared and ready for class, it is recommended that you purchase your textbooks and supplies no later than August 15th.
