  IRS Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 tax year is available upon request. This form provides information you may need to file your income tax return.
  
  You may request a copy of your 2025 Form 1095-C by contacting us at:

  Please include your name and, if possible, employee ID number in your request. Copies will be provided within 30 days of receiving your request.
  
  This notice will remain available through October 15, 2026.
If you wish to make any changes to your current plan, you will need to complete a 1-page change form and forward it to the Benefits Coordination Office
Any changes to your health, dental or vision plans must be received within 30 days of your Qualifying Life Event.

March is National Brain Injury Awareness Month, led by the Brain Injury Association of America (BIAA), to educate the public, reduce stigma, and support over 5.3 million Americans living with permanent brain injuries. The 2026 campaign features the theme “My Brain Injury Journey,” focusing on sharing stories, promoting prevention, and highlighting recovery resources.

Key Aspects of Brain Injury Awareness Month:

Purpose: To raise awareness, educate on causes (TBI and ABI), and support individuals affected by brain injuries.
Theme: The #MoreThanMyBrainInjury campaign emphasizes that individuals are not defined by their injuries.
Prevention: Focuses on reducing risks through safety measures like helmets, seatbelts, and preventing falls.
Action: Involves advocacy, fundraising for research, and sharing personal stories.

What is a Brain Injury?
An acquired brain injury (ABI) is damage to the brain that occurs after birth and is not hereditary or degenerative. It includes:

Traumatic Brain Injury (TBI): Caused by an external force (e.g., falls, accidents).
Non-Traumatic/Acquired Injury: Caused by internal factors (e.g., stroke, infection, lack of oxygen).

Educational Events: Participate in webinars and conferences, such as the 2026 Kickoff Webinar.
