At the bottom of this page, in the “Related Files” area, you will be able to access the Annual Professional Performance Review (APPR) Plan in its entirety, as well as all of the documents associated with the 3012d APPR process.

The 3012d APPR Process Chart and 3012d Block Diagram of Activities depict the process.  These documents are designed to help guide you through the process.

The Appendices (documents and forms) used by the 3012d APPR process are listed in Appendix alpha order.  To use them, they MUST be downloaded and saved to your computer, where you can then complete, save, edit, and print them.

Reference materials include the 3012d APPR Appeals Process for Teachers, an Appeals Tracking Chart, and the complete 2013 Danielson teacher rubric.
